As Elder Care Managers, Touching Hearts Senior Care works with older adults and their loved ones to develop a plan of care and services that promotes autonomy, independence and keeps them safe.
This may include:
- Living arrangement placement.
- Arrange and monitor in-home help.
- Assessment and evaluation of non-medical needs
- Coordinating home care, transportation, educational opportunities and socialization

PATIENT NAVIGATION
You or a loved one has just been released from the hospital. Now what?
We are expert in helping clients navigate the health care system.
We can help you identify and prioritize your unique needs such as:

We make the calls and gather the information on your behalf. We’re very persistent.
- Accessing community resource
- Coordinate and attend doctor appointments.
In an effort to help our clients and their families make informed decisions regarding their care we accompany the client to make sure that no pertinent information is ignored.
DAILY MONEY MANAGEMENT
At Touching Hearts Senior Care, we encourage individuals to remain in control while receiving the right amount of support in an effort to avoid errors that could lead to both financial loss and loss of independence.

- Paying bills and resolving billing discrepancies
- Accessing public and private benefits and entitlements

FAMILY MEDIATION
We work with the older adult and his or her loved ones to facilitate communication and informed decision making. This process can help avoid expensive and emotionally taxing court involvement.
Voluntary mediation can help families confidentially settle disputes concerning:
- Care assessment and assignment of responsibilities
- Living arrangements
- Who should be appointed power of attorney, health care proxy, etc.
- How finances should be managed
- Estate and trust planning and administration
